I hereby authorize Campus Outreach Greenville to transfer the amount listed above from my bank account each month on or after the date provided. This authority will remain in effect until I provide written notice to cancel this agreement.

Upon receiving your EFT form, we will call you to obtain your bank account and routing numbers, so that they are not compromised for security. Then, once your pledge is processed, we will send you a letter confirming your pledge amount and detailing our management of donor contributions.
